Position Summary:
As an RN Clinical Care Coordinator, you will play a crucial role in coordinating clinical care for ALWP clients under the Care Coordination Agency (CCA). This involves identifying, organizing, coordinating, and monitoring necessary services for recipients. You will assist waiver recipients in accessing waiver services, state plan services, and community resources, acting as a vital clinical resource for the CCT Program.
Qualifications:
Current, unrestricted Registered Nurse license in California.
Familiarity with the State of California Assisted Living Waiver Care Coordination Agency Provider Handbook and RCFE Provider Handbook, and the clinical requirements of the CCT and ALWP transition processes.
Knowledge of the Nursing Practice Act and CPR certification.
Proficiency in computer use.
Minimum two (2) years of experience in home health, primary care clinic, or health facility settings, with at least one (1) year in healthcare quality management preferred.
Clear pre-employment physical examination report, including TB screening or chest X-ray.
Strong clinical judgment, analytical skills, and documentation proficiency.
Excellent written and verbal communication skills in English.
Valid driver's license, automobile insurance, and reliable vehicle as per state and organizational requirements.
Essential Duties and Responsibilities:
Ensure accurate completion of clinical documentation for State Waiver Program Applications and CCT enrollment.
Conduct assessments using the ALW Assessment Tool to determine client care levels.
Develop and modify Individualized Service Plans (ISPs) using the ALW ISP Form.
Monitor service delivery according to client contact schedules, conducting face-to-face visits as required.
Maintain comprehensive charting, progress notes, and case records for all clients.
Address and resolve client, family, or friend complaints promptly.
Report all instances of abuse or neglect to appropriate authorities.
Attend regular Case Conferences with Residential Care Facility staff.
Evaluate ISP effectiveness through client observation and communication with healthcare team members.
Review Significant Incident Reports (SIRs) with clients and facility staff as needed.
Provide clinical support to CCA Office Coordinators.
Participate in education, training, and quality improvement activities for the ALWP, ensuring compliance with regulatory standards.
Foster positive relationships with Residential Care Facilities (RCFEs), Skilled Nursing Facilities (SNFs), and other vendors.
Assess and enhance nursing skills and educational needs to meet patient care requirements.
Support assessments and clinical needs across various departments.
Travel to SNFs and RCFEs to conduct assessments and complete necessary paperwork.
Perform additional duties as assigned.
